Innova Physical Therapy Rates & Policies

Rates/Policies

At this time Innova Physical Therapy is in the process of going out of network with insurance companies. We are in network with the following insurance until the date listed:

• Aetna 11/30/25
• CDPHP- Medicaid and Essential plans 10/22/25. Commercial and Medicare Advantage 12/31/25
• Anthem Blue Cross (awaiting date)
• Blue Shield (Federal and Highmark) 10/7/25
• Medicare
• MVP 10/15/25
• UHC 9/7/25

For all others and after the above dates, we are a fee-for-service provider. Payment is due at your time of service and we will provide you with an itemized receipt that you may try to submit to your insurance company for reimbursement. It is always best to call your insurance company first and check your coverage for out-of-network physical therapy. Many insurance companies will often provide an exception and cover you fully at Innova because of the specialty that we provide in pelvic floor dysfunction. Rates

• $175 Hour long initial evaluation
• $75 for 30 min session

Cancellation

Arriving to your appointments and completing your home exercise program are the keys to your success in physical therapy. I understand that emergencies occur and you are sometimes unable to make it to your appointment. Please allow 24 hour cancellation notice if you are unable to attend your session. This allows time for you to reschedule your appointment and allows another patient to obtain that treatment time. There is a $75 cancellation fee for cancellations less than 24 hours.
